In the old days, each individual, beginning in children, learned how to take care of oneself, to be self-reliant and to maintain pono relationships.  If sick, one looked within oneself first to see what one had thought or said or done that might be responsible for the imbalance.  Then one attempted to correct it.  If the illness persisted, then one went to the next level -someone more knowledgeable or skillful, usually an elder within the 'ohana.  If not resolved at that level, and one had the resources to go to the next level, then one went to the kahuna (medical expert) at the nearest heiau ho 'ola.
